Our integrated services focus on addressing all healthcare needs and removing unnecessary barriers to care.

It takes teamwork and a shared vision. MHC clients have a highly-skilled treatment team that includes a care manager, psychiatrist/psychiatric nurse practitioner, registered nurse, care management supervisor, and behavioral health clinicians. Our multidisciplinary team collaborates to address each client's needs and treatment goals.

Community-Based Care Management

Care managers provide side-by-side support to clients so they can enjoy a fulfilling and healthy life in the community. Care managers work to remove barriers to care and help connect individuals to a wide variety of resources, such as:

  • Budgeting and Financial Benefits
  • Medical and Dental Care
  • Vocational and Educational Services
  • Social Support
  • Transportation Resources

Psychiatry

Psychiatrists and psychiatric nurse practitioners provide comprehensive psychiatric evaluations and treatment recommendations that may include ongoing medication management.

Therapy

Therapists provide counseling using trauma-informed, evidence-based models, including CBT, TF-CBT, EMDR, DBT, and Motivational Interviewing.

Intensive Care Management

Depending on your needs, MHC offers varying intensity levels of targeted in-home care management, including CTT and PACT.
